These forms are all required before counseling sessions can begin. Teenagers are welcome to fill out as much as they can, but ultimately, parents will need to be involved with the contract and insurance declarations for their non-emancipated teens. The “Teen History” form is the exception. This form should be completed by the teenager seeking services and kept privately by the teen until their session.
Parents and teens should all be aware that counselors at North Shore Counseling are mandated reporters. We are legally obligated to alert the authorities when anyone is in imminent risk of harm to self or others.
